La Commission nationale de l'informatique et des libertés doit être saisie, préalablement à toute mise en oeuvre de fichiers constitués au niveau départemental, d'une déclaration comprenant :

-un formulaire de déclaration rempli au recto ;

-un engagement de conformité au modèle type national (n° M 107.406) précisant notamment les conditions d'exercice du droit d'accès et de rectification ;

-une annexe relative aux mesures prises pour assurer la sécurité et la confidentialité du traitement.
