Cap. 16 Art. 293Particularsofactof marriage.

Where any marriage takes place, the parties contracting such marriage shall draw up or cause to be drawn up an act, in accordance with Form E in Part II of the First Schedule to this

Code, entering therein -

(a) the date of the act;

(b) the name, surname, date and place of birth, a legally valid identification document and place of residence of the parties;

(c) the name, surname, date and place of birth and place of residence of the witnesses present at the solemnization of the marriage;

(d) he name and surname at birth and after marriage of the parents of the parties;

(e) the day, month and year when, and the church, chapel, or other place where the marriage took place;

(f) a declaration as to the solemnization of the marriage signed by both of the parties, or if the marriage takes place by proxy by the proxy and by the other party, in the presence of and countersigned by an officer of the Marriage Registry or other person authorized for the purpose by the Marriage Registrar; and

(g) the surname after marriage of both spouses, in accordance with article 4(1).
