When the midwife’s attendance is required away from the town, suburb, or village in which she resides, for every mile or part of a mile from such town, suburb or village to the limits of the town, suburb or village where the patient resides, there shall be reckoned additional to the fees laid down in this tariff and to the travelling expenses according to law, a rate of .......................... .12
