S.L. 94.13 чл. 41Form of application.

(1) An application for registration in any of the registers shall be made to the Registrar in accordance with the form shown in the Second Schedule and, unless the Board otherwise direct, shall be accompanied by the licence granted to the applicant by the Minister responsible for Health.

(2) Such licence shall be returned to the applicant by the Registrar as soon as the Board has determined upon the application.
