(1) Subject to the provisions of this regulation, where on the arrival of a ship, the master -

(a) makes a report in accordance with regulation 14(1)(a),

(b) or (c), or

(b) is directed by the medical officer to complete a Maritime Declaration of Health, he shall complete a Maritime Declaration of Health in the form set out in Schedule 1, which shall be countersigned by the ship’s medical officer.

(2) The master shall deliver such Declaration to the local authority.
